fix(ai): 修复提供商切换与 Codex 自定义端点

- 分离 OpenAI-compatible 与 Codex CLI 的模型和推理配置
- 复用本机 Codex provider、认证与自定义端点,并适配 Docker 回环地址
- 隔离 OpenAI 专属 reasoning_effort,修复自定义预设切换与表单保留
